Instructions for setting up a payment plan:
- Print and submit the following payment plan form (PDF). The form must be signed in front of a notary. You can submit it by email, fax, mail or in person.
- Pay the initial down payment by internet,mail, or money order, in person with your signed payment plan.
- Once the Clerk receives your down payment and notarized form, the Clerk will update your case(s) to reflect the payment agreement.
- If you would like a copy of your plan or receipts for any payments, you must send a self-addressed stamped envelope to our office with your payments.
